Priya Bayyapureddy, MD
Dr. Priya Bayyapureddy is a compassionate board certified Internal Medicine doctor. She is certified as a diplomat in Obesity Medicine by American Board of Obesity Medicine (ABOM) and is a member of American Society of Bariatric Physicians (ABSP). She has been recognized as "Leading Physician of The World" by the International Association of Internists. She has been appointed as Civil Surgeon by the U.S. Citizenship and Immigration services. She is a member of International Association for Physicians in Aesthetic Medicine. She started Windermere Medical Group to achieve her goal to be a community physician for all primary health care needs for families. During her career, she worked for VA Clinic and served as part time faculty member at Emory University School of Medicine. She provided Occupational and Travel Medicine consulting services to Centers for Disease Control and Prevention (CDC-Atlanta). She completed her Internal Medicine residency at Emory University School of Medicine and internship at University of Tennessee College of Medicine at Chattanooga. She has hospital privileges at Northside Hospital - Forsyth & Cherokee and Emory Johns Creek Hospital. She speaks English, Hindi and Telugu. Dr. Bayyapureddy has been honored with Patients' choice and Most Compassionate Doctor awards for several years.
Indra Shah, MD
Indra Shah, MD is a friendly and compassionate primary care doctor. Dr. Shah has over 20 years of experience in Primary care and completed training in internal medicine at Edgewater Hospital & Saint Joseph Hospital. Dr. Shah is board certified in Internal Medicine and Geriatric Medicine by the American Board of Internal Medicine. Dr. Shah is a member of the American College of Geriatrics Specialists. Dr. Shah previously served as Chairman of Department of Internal Medicine and Medical Director of Transitional Care at Tift Regional Medical Center. Dr. Shah speaks English, Hindi and Gujarati.
AnnMarie Stokes, MD
AnnMarie B. Stokes, MD is Board Certified in Family Medicine as well as Obesity Medicine. The last 23 years of her professional career have been diverse with experiences in Primary Care, ER/Urgent Care, and Occupational Medicine. Dr. Stokes attended Medical and Pharmacy school at the University of Mississippi Medical Center in Jackson MS and did her residency at North Mississippi Medical Center in Tupelo MS. She served as a Flight Surgeon in the US Air Force.
Jeff Buchanan, PA-C
Jeffrey D. Buchanan, PA-C is a graduate of the University of Michigan and a former United States Marine Corps fighter pilot. He attended Emory University School of Medicine and received his Physician Assistant training there graduating in 1998. Jeff served as a US Public Health Service, National Health Service Corps PA, providing medical care to under-served populations, is currently working in the Emergency Department of Oconee Regional medical Center and comes to us with a wealth of practice experience in emergency, family practice and adult medicine. He is certified by the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants.
Katy Hanaford, FNP
Katy Hanaford, FNP, earned her Master's of Science from the University of North Georgia. She is a member of the American Association of Nurse Practitioners. As a 1998 graduate of Kennesaw State University with her Bachelor in the Science of Nursing, she has 17 years’ experience in medical, surgical, cardiology and emergency medicine. Her interests include cardiology and preventative medicine. She lives in Ball Ground with her husband and 4 children.
Dana Hickman, FNP
Dana Hickman, FNP, earned her Master's of Science in Nursing from University of North Carolina at Chapel Hill in 1988. With over 25 years of experience as FNP, Dana is well known in local medical community. She held various clinical and leadership positions during her career in primary care settings, hospitals and insurance companies. She lives in Cumming with her husband.

How long will it take me to get an appointment?
Usually same day appointments.
What is the typical wait time?
Usually 15 to 20 min.
What do you charge for Co-Pay?
For insured patients we collect normal Physician office (Primary Care) standard co-pay for any visit. We do not collect Urgent Care co-pays that are usually high and we do not charge any convenience fee for weekend and evening hours like other doctor's offices. We keep it simple and provide excellent medical care at lowest cost to you.
When should I have a complete physical?
You should have one once a year, every year.
What days are you open?
Our Windermere Medical office is open 6 days week. Our Canton Primary Care office is open 5 days a week.
What services do you provide?
We provide Primary Care and Urgent Care services for all individuals and families.
Can I get a copy of my lab results?
Yes, we will be glad to give you results. We can make these results available for you online using patient portal or Fax or pick up at our location. Please contact us for details.
Who do I call if I have a questions about my bills?
We are affiliated with Northside Hospital and part of its Primary Care Services. Patients can pay their bill online. Our Northside billing office can be reached at (678) 223-7932 Monday to Friday 9am to 5pm. Patients can also email email@example.com with billing concerns.
Do you provide Financial Assitance for Patients?
As an affiliate of Northside Hospital, Inc. we are committed to fulfilling Northside's charitable mission as a health care provider. Uninsured, underinsured and Medically Indigent Patients having limited or inadequate resources to pay for Medically Necessary Services rendered at a our Practice may be eligible for full or partial Financial Assistance through Northside’s Physician Practice Financial Assistance Program "PPFAP".
What is your Medication Policy?
It is the policy of our office to not phone in ANY controlled substance to your pharmacy. Maintenance medications will not be phoned in unless you have been seen in this office in the last six Months. Please allow 2-3 business days for any maintenance medications to be refilled upon notification in this office. Our goal is to provide the best medical care to meet your needs and this policy is in place to adhere to the high medical care standards we strive to provide.
What is your Forms Policy?
It is the policy of this office to charge a $20.00 fee for each occurrence of any medical forms needing to be completed. (i.e. disability forms, physical forms, insurance forms, etc.)
